Adult Congenital Heart Program
Adult Congenital Heart Disease Program at Kings & Queens Teaching Hospital The KQTH Adult Congenital Heart Program treats patients with congenital heart disorders. We also host congenital heart disease clinics twice each week. KQTH strategic approach to treating adults with congenital heart defects combines the expertise of our top pediatric cardiologist with that of an adult cardiologist specially trained in treating congenital heart disease. Our congenital heart disease program was created in response to growing concern among cardiologists across the country that many adults with congenital heart defects may not realize that they need continuous specialty care. Adult congenital heart patients can be divided into three main groups: • Simple congenital heart defects: These were almost completely repaired but still linger as potential problems later in life. • Complex congenital heart disorder problems: These are often found in adults who were born with congenital heart defects that were completely repaired. They typically lead normal lives until they develop problems such as arrhythmia or heart murmurs in their 20s or 30s. • Non-diagnosed congenital heart disorders The numbers of Ghanaian patients with congenital heart defects are increasing with advances in care. Today, about half of congenital heart disease patients are over 18 years old. Before the 1980s, most children with congenital heart problems died. About 1000 children are born with heart disease each year in Ghana. That's about one percent of the population. About half of them have significant issues. Based on those figures, it is estimated that 5,000-8,000 adults with congenital heart disease in Ghana need ongoing care.

Patient and family-centered care is not just about “being nice” or “patient satisfaction”. It is an approach to the planning, delivery and evaluation of health care that is grounded in mutually beneficial partnerships among healthcare providers, patients and families. Patient and family centered care is an approach to healthcare that shapes policies, programs, facility design and day-to-day interactions of staff. What are the core concepts of patient and family centered care? • Respect and dignity: We listen to and honor patient and family perspectives and choices. The expertise, preferences, and culture of each individual and family are valued. • Information Sharing: We communicate and share complete and unbiased information with patients and families in ways that are affirming and useful. Patients and families receive timely, complete, and accurate information in order to effectively participate in care and decision-making. • Participation: Patients and families are encouraged and supported in participating in care and decision-making at the level they choose. • Collaboration: We collaborate with patients and families in policy and program development, implementation, and evaluation; in health care facility design; and in professional education, as well as in the delivery of care. Patient and Family Centered Care recognizes that the relationship between patients, families and providers is an essential collaboration. This collaborative approach to care leads to better health outcomes, wiser allocation of resources, and greater patient and family satisfaction What is meant by the word "family"? The word "family" refers to two or more persons who are related in any way—biologically, legally, or emotionally. Family is defined by the patient. Family members are encouraged to be involved and are essential to the overall well-being of the patient. Visitors are guests of the patient or family. In some cases, visitors may be relatives. KQTH Patient Family Partnership Councils Individuals serving as patient or family partners should reflect a range of experiences within the hospital and should be representative of the community served. Council members must be able to: • Share insights and information about their experiences in ways that others can learn from them. • See beyond their own personal experience and show concern for more than one issue or agenda. • Listen well and respect the perspectives of others. • Speak comfortably and candidly in a group. • Interact well with many different kinds of people and work in partnerships with others. Alzheimer's disease (AD), also known as Alzheimer disease, or just Alzheimer's, accounts for 60% to 70% of cases of dementia. It is a chronic neurodegenerative disease that usually starts slowly and gets worse over time. The most common early symptom is difficulty in remembering recent events (short term memory loss). As the disease advances, symptoms can include: problems with language, disorientation (including easily getting lost), mood swings, loss of motivation, not managing self care, and behavioural issues. As a person's condition declines, she or he often withdraws from family and society. Gradually, bodily functions are lost, ultimately leading to death. Although the speed of progression can vary, the average life expectancy following diagnosis is three to nine years. The cause of Alzheimer's disease is poorly understood. About 70% of the risk is believed to be genetic with many genes usually involved. Other risk factors include: a history of head injuries, depression or hypertension. The disease process is associated with plaques and tangles in the brain. A probable diagnosis is based on the history of the illness and cognitive testing with medical imaging and blood tests to rule out other possible causes. Initial symptoms are often mistaken for normal ageing. Examination of brain tissue is needed for a definite diagnosis. Mental and physical exercise, and avoiding obesity may decrease the risk of AD. There are no medications or supplements with evidence to support their use. No treatments stop or reverse its progression, though some may temporarily improve symptoms. Affected people increasingly rely on others for assistance, often placing a burden on the caregiver; the pressures can include social, psychological, physical, and economic elements. Exercise programs are beneficial with respect to activities of daily living and potentially improve outcomes. Treatment of behavioral problems or psychosis due to dementia with antipsychotics is common but not usually recommended due to there often being little benefit and an increased risk of early death. In 2010, there were between 21 and 35 million people worldwide with AD. It most often begins in people over 65 years of age, although 4% to 5% of cases are early-onset Alzheimer's which begin before this. It affects about 6% of people 65 years and older. In 2010, dementia resulted in about 486,000 deaths. It was first described by, and later named after, German psychiatrist and pathologist Alois Alzheimer in 1906. In developed countries, AD is one of the most financially costly diseases.
