Your privacy is important to us. Kings and Queens University Teaching Hospital will use your confidential medical information in order to provide you health care services. Kings and Queens University Teaching Hospital takes many precautions to maintain your confidential medical information in strict confidence and will not deliberately disclose any information regarding you to any unaffiliated third party unless you authorize that person to receive your information or the information is permitted to be disclosed by law. All messages sent and received within MyKQTHChart that contain medical information are subject to all state and federal laws governing the security and confidentiality of medical records.

The acceptance of cookies is not a requirement for visiting the Kings and Queens University Teaching Hospital website. A. Use of MyKQTHChart

NEVER send any messages requiring urgent attention using MyKQTHChart. Physicians and staff do their best to read and respond to requests within three business days. However, delays can occur based upon volume and availability of staff. Therefore, for all urgent medical matters, contact your health care provider's office immediately, go to an emergency room, or call +233-247176522.

B. Eligibility

Two requirements must be met for a patient to be eligible to participate in KQTHMyChart. First, you must be 18 years or older. Second, you must be an active patient of a Kings and Queens University Teaching Hospital  health care provider who has elected to offer MyKQTHChart to his/her patients. IMPORTANT NOTE: Not all Kings and Queens University Teaching Hospital Physicians will choose to communicate using MyKQTHChart, in which case communication will be through Email service or telephone.

C. Activation Code

MyKQTHChart is designed as a secure Internet-based environment through which you may receive confidential medical information about yourself. Once you sign up for MyKQTHChart, you will be given a one-time-only MyKQTHChart "Activation Code" that you will use to activate your account. You will then create your own MyKQTHChart username, password and password hint. Please DO NOT SHARE your password with anyone. The individual MyKQTHChart username and password you choose when you first log-on to MyKQTHChart are the keys to protecting the confidentiality of your information. You are responsible for remembering your MyKQTHChart username and password and for keeping them confidential.

  1. From the time you receive your temporary, one-time use Activation Code, you will have ONLY 90 days to log-on and activate your MyKQTHChart account. After 90 days, if you have not activated your account, your temporary ten-digit "Activation Code" will expire. PLEASE activate your account as soon as possible so that you may begin using the services immediately, and not risk the inconvenience of having to sign up a second time.
  2. After you have activated your MyKQTHChart account, MyKQTHChart will become the primary vehicle through which you will receive important messages from your primary care health care provider and selected test results and other personal medical information. You will continue to receive communications via the Ghana Postal Service or other means from health care providers that have opted to not use MyKQTHChart.
  3. Through MyKQTHChart you may request new appointments and prescription renewals 24 hours a day, 7 days a week. We ask that you first contact your pharmacy for existing, non-narcotic, prescription refills. Physicians and staff do their best to read and respond to requests within three business days. However, delays can occur based upon volume and availability of staff. Therefore, for all urgent medical matters, contact your health care provider's office immediately, go to an emergency room, or call +233-247176522.
  4. You MUST provide us with your email address when activating your MyKQTHChart account and keep your email address current via the "Personal Preferences" link in MyKQTHChart. No confidential information will be sent to your email, but you will receive email notification whenever a new result has been released to your MyKQTHChart account. You therefore MUST verify and update your email address via the "Personal Preferences" link to ensure that you receive notification of newly released information in a timely manner.
  5. Once you have activated your account, your patient header display will include an active MyKQTHChart designation. You will then receive your personal health information as released by your health care provider in your personal MyKQTHChart account.

D. Electronic Messaging

  1. Your health care provider may choose to select an associate to help manage and respond to your messages. These may include nursing or other administrative staff who review messages sent via MyKQTHChart and either respond or route the message to the health care provider. Therefore use of MyKQTHChart secure messaging may not be appropriate if the message includes sensitive information that you would only wish to discuss directly with your provider.
  2. All communications between you and your Kings and Queens University Teaching Hospital  health care team using MyKQTHChart are carried over a secure, encrypted connection directly into the Kings and Queens University Teaching Hospital  electronic medical record. While you may receive Internet e-mail messages notifying you of new messages in your MyKQTHChart Inbox, these e-mails will not contain any confidential medical information.
  3. Messages you send via MyKQTHChart may become part of your permanent legal medical record at the discretion of the physician or support staff. Once a message is made part of your permanent medical record, it will be accessible to current and future Kings and Queens University Teaching Hospital health care provider(s) and staff members who are appropriately involved with your care.
  4. You understand that when you receive messages, test results and other communications delivered via MyKQTHChart, you will not receive duplicate written reports of this information from Kings and Queens University Teaching Hospital.

E. Medical Record Data

  1. As we provide your health care, we are required to maintain documentation of your medical history, current conditions, treatment plan and all treatments given, including results of all test, procedures and therapies.
    NOT ALL of your Kings and Queens University Teaching Hospital medical record may appear on your MyKQTHChart. Posting of information is at the discretion of your individual health care providers(s) and Kings and Queens University Teaching Hospital.
  2. Your health care provider(s) will maintain and update your electronic record at each office visit. However, if you believe any information displayed in MyKQTHChart is not accurate, you are encouraged to notify your health care provider at your next office visit or submit a message to your clinic.
  3. The Kings and Queens University Teaching Hospital reserves the right in the future to charge a fee for the use of MyKQTHChart. You will be notified of this change and asked to re-apply if you agree to the fee and wish to continue with this service.

F. Information Does Not Constitute Medical Advice

Kings and Queens University Teaching Hospital assumes no responsibility for how you use the information you obtain from this Site. You should consult with a physician or other health care provider regarding your own condition and how this Site content may or may not apply to you. You may be accessing your own medical information via the MyKQTHChart application and may wish to communicate with one of your health care providers about that or related information. Neither MyKQTHChart nor any other application available through this Site is a substitute for appropriate and timely contact with your health care provider. This Site is not a substitute for consultation with your health care provider. You should never change or stop any course of treatment prescribed by your health care provider without first consulting him or her.

G. Termination of Access

Kings and Queens University Teaching Hospital may restrict or terminate your access to MyKQTHChart at any time. If access is restricted for planned maintenance of the MyKQTHChart system, every effort will be made to communicate downtime to all users in advance. However, there may be reasons to restrict access to MyKQTHChart without advance notice. If your use of MyKQTHChart violates these Terms and Conditions, Kings and Queens University Teaching Hospital reserves the right to permanently terminate your access to MyKQTHChart.

H. Restrictions on Use and Ownership

Requesting Copies of Your Medical Records

Requests for medical records must be made in writing. Please follow these steps.

Important Information About Requesting Medical Records

Records can be released to anyone who the patient authorizes (in writing) to receive such information. The authorization form above can be used to request records for personal use or for continuing medical care. You can submit the same form for our Kings and Queens University Teaching Hospital locations.

A valid authorization MUST contain the following information or the request will be returned:

Patient’s full name and date of birth (list any other names the patient may have had)

Medical record number (if available)

Specific information being requested (i.e., type of report/information and dates of service, etc.)

Purpose for which the information may be disclosed (i.e., personal use, continuity of care, legal matter)

To whom the information is to be sent (name and address)

Specify authorization’s expiration date if desired (otherwise, the authorization will be valid one year from date signed)

The patient’s signature or a patient’s legal representative’s signature. Authorizations signed by a representative must be verified. Please include a copy of one of the following documents indicating either:

Legal guardianship papers, or

Advance Directive/Healthcare Power of Attorney (Download in English), for patients unable to make healthcare decisions, or

Designation of Personal Representative Form, (Download in English) which allows the representative to act on the patient's behalf with regard to personal health information.

Please note that unsigned requests will not be processed

Date of the signature

Requests for medical records of deceased patients require a copy of the death certificate or evidence of next of kin or executorship of the estate.

Please also include your phone number in case we need to contact you for additional information concerning your request.

Requests for Personal Use

There is a fee for the copies and processing of the copies if the request is for your personal use.

Please allow reasonable time to process your request. We will contact you in the event we experience unforeseen delays or are unable to fulfill your request.

Records will be mailed to the address specified on the authorization form, or you may pick them up at our office if you make arrangements with the Release of Information Staff. For security reasons, please be prepared to show proper photo identification.

Requests for Continuing Medical Care

Medical emergencies will be faxed free of charge directly to a physician or medical facility.

Continuing care requests are also free of charge and will be mailed to your clinic/physician(s) prior to your appointment (please indicate the date of your appointment on the authorization form so that the copies are received early enough for your physician to review).

Pertinent information such as radiology/imaging, history and physical, consultations, operative reports, and discharge summaries are routinely provided to the physician for continuing care.

Sensitive Information

Certain information requires a special authorization covering sensitive information. This includes psychiatric, drug and/or alcohol abuse, HIV/AIDS, genetic testing. Authorizations for sensitive information must specifically refer to the information that is to be released.

PRICING, CANCELLATION AND REFUND POLICY OF KINGS AND QUEENS UNIVERSITY TEACHING HOSPITAL LTD

PRICES AND PAYMENTS OF HOSPITAL BILLS

All prices paid within Kings and Queens University Teaching Hospital are Quoted in Ghana Cedi (GHC) unless otherwise stated.. The total cost of your order will include the service fees, VAT, NHIS Levy. Payment is deducted once an service is rendered or order is submitted.
Payments

Payments are accepted via Credit Card through Visa Card and Master card ONLY and Accepted in Ghana Cedi ( GHC) or US Dollar ( USD$ )Equivalent at the Current Exchange Rate

 Payment is due for receiving after 5 days of assessing Hospital services. If payment is not received within 5 days of the due date,   Kings and Queens University Teaching Hospital reserves the right to suspend any and all related services for client.

For services , payments may be split into multiple payments, however this option will only be offered at Kings and Queens University Hospital’s  discretion.

Price list of all Hospital Services offered by Kings and Queens University Teaching Hospital is at the Accounting section of the Hospital and on Notice Board while other services will be posted on Hospital Website

When deciding whether to accept or make your order or payment, we may use certain information about you, including any received from our accredited identity verification partner. This will mainly be verification from your bank behind the scenes to authenticate the card holder. For example, we may pass on your details for your bank to protect any fraudulent activities.                                                   

Unless expressly stated otherwise all prices are exclusive of Value Added Tax and NHIS Levy. The donated amount to be paid to Kings and Queens University Teaching will be given consideration that is governed by or connected with these Terms. Only Transaction fees are deducted from the total amount for Kings and Queens University Teaching Hospital by the Gateway Merchant Processor.

30 Day Money Back Guarantee

In the unlikely event that you are not completely satisfied with our service, we are happy to provide you with a full refund for the cost of your payment or donations. What if I overpay my bill?

Patient Financial Services will work with you to resolve any billing discrepancies that may occur. If there is an overpayment to your account, it will be transferred to other open self-pay outstanding balances (Kings and Queens University Teaching Hospital). The refund of any remaining overpayment will be sent to the patient/guarantor. If the overpayment is due to a payment from a credit card, the first priority will be to send the overpayment back to the credit card issuer; otherwise, a check will be issued to the patient/guarantor. Please allow 4-6 weeks for the overpayment to be processed.
